First, we listen to your concerns.  We gather information to determine how we can help.  We continue to explore the situation to determine if the frail elder will cooperate with changes that may need to be made. 


We are knowledgeable of and connected to high quality resources in the community for care of the elderly.

     Coordinate community services such as home care, home health care, adult day care and lifeline (PERS, Personal Emergency Response program).

     Arrange for physician and dental appointments, and accompany the client to same when requested by the family.  Sometimes we stay with a client in the Emergency Room, to offer comfort and familiarity and communication.

     Help selecting an appropriate congregate living home: a Retirement Community, an Assisted Living Home, or a Nursing & Rehabilitation Center.


 

We empower the family to make important decisions.

     Provide long-distance liaison for separated families on a 24/7 basis.  Personal visits and telephone reassurance are offered to the client as needed.

     Accept designation as agent for Power of Attorney for Health Care (POAHC), then provide sensitive and supportive counsel in implementing end-of-life decisions.  Engage Hospice when directed to do so by the family.


We pay attention to critical business issues.

    Consult with financial and legal professionals (taxes, estate planning, wills).

     Monitor monthly checking account, insurance claims, tax preparation.  Share current knowledge of entitlements, including Medicare and Medicaid.


 

We help families make those difficult transitions.

     Escort services for families that need to move a frail elder (by plane or car) from a distant location.

     Help individuals downsize, or families to distribute an estate, by serving as agent to organize and implement the process.

We pride ourselves in offering personalized and gentle service, either short-term or as on-going assistance, uncomplicated by bureaucratic red tape and unnecessary formality.

We consider aging to be a family affair.  While our service is for the benefit of the frail elder, we are always cognizant of the needs and feelings of the family, care providers and support system.
